Neither dependency nor addiction applies strictly to substance use, as both terms describe complex biological, psychological, and behavioral processes that can involve many different objects, substances, or routines. Dependency is a specific clinical term describing a physiological state where your body adapts to a substance so thoroughly that it requires it to function, experiencing acute, life-threatening withdrawal if it is abruptly stopped. This physiological adaptation is extremely common with Benzodiazepines and Alcohol. Addiction, by contrast, is the psychological and neurological alteration of the brain's reward system, driving compulsive seeking behaviors even when the social, bodily, or relational cost becomes overwhelming. This reward system shift can occur with substances, but it also applies to non-substance behaviors like compulsive sexual patterns, where people adopt the label of sex addict because the behavior has become the sole substitute for genuine human intimacy.
Clarifying these terms is essential for defining my role and scope of care. I am not an inpatient detox facility, a residential treatment center, or a medical clinic equipped with emergency medical services, and I cannot prescribe or manage psychiatric or detox medications. If physical dependency arises in our work, I am fully comfortable working alongside you and advocating directly with your medical care team. Going through physical detox is terrifying not just because medical facilities can feel cold or coercive, but because you are losing something your physical body has relied on to survive. In my practice under Substance Use & Addiction, we address the profound emotional and relational void that opens up when the body or mind is stripped of its primary coping mechanism. Detox can be very scary and not just like, oh I may die or this is a scary facility with scary rules, as in, you are losing a thing your body was dependent on. Thats a hard spot to be in. This is where I want to link back to Drug, Alcohol, and Addiction to talk about standards of care when treating addiciton.
