Tourettic OCD sits at the precise intersection where motor-neurological tics and compulsive behaviors bleed into each other. Standard OCD is typically driven by cognitive anxiety and feared catastrophic outcomes which is the belief that if you do not perform a ritual, something terrible will happen. Tourettic OCD is different. It is driven by a visceral, physical imperative. This is called a premonitory urge, a somatic itch, or an internal sensory imbalance that will not quiet down until a movement, touch, or sequence feels physically "just right".
In my own life and practice, I know that sensorimotor compulsions and tics are deeply somatic. It is not an abstract fear in your head; it is a physical sensation in your muscles, skin, and nervous system.
When you live with Tourettic OCD, your body demands physical completeness. The urge might compel you to tap a surface until the vibration feels even in both hands, clench specific muscle groups, blink until a sharp eye strain resolves, or repeat a movement until the physical discomfort clears.
Traditional Exposure and Response Prevention (ERP) often fails people with Tourettic OCD because standard clinicians treat sensorimotor urges purely as cognitive anxieties. Telling someone with Tourettic OCD to simply "sit with the anxiety" while ignoring an unbearable physical imperative - or treating the sensation like an irrational thought - is ineffective and dogmatically cruel. Forced suppression of physical urges can spike bodily anxiety to the point where your surrounding environment feels like a thousand buzzing flies crawling across your skin.
I do not force painful behavioral suppression, nor do I treat your physical urges as behavioral flaws that need to be extinguished. We work directly with your nervous system's sensory processing and bodily realities.
In our work together, we focus on untangling pure involuntary motor tics, premonitory sensorimotor urges, and fear-driven compulsions without clinical shaming; finding non-destructive body-honoring ways to navigate sensory incompleteness and intense physical discomfort; addressing the real physical toll, muscle strain, and fatigue caused by repetitive physical movements and sensorimotor loops; offering relational co-regulation and grounded support rather than demanding you endure unbearable somatic overload alone; and backing your choices around pharmaceuticals, working alongside prescribers, and securing formal accommodations when physical symptoms impact school or work.
