In traditional mental health systems, clinical records often function as instruments of diagnostic surveillance and institutional liability management. My practice takes a fundamentally different approach. Documentation is treated strictly as a regulatory necessity, and record-keeping is intentionally designed to protect your privacy and to build documentation that serves a human's needs. My records are like a key for me to use to unlock doors for you.
Your notes do not sit on cloud-based servers or vulnerable corporate electronic health record (EHR) platforms. I maintain records by hand in physical journals, secured inside a 200-pound fire safe in my physical office. Because legal and state systems can technically subpoena records, my charting is written with that threat in mind. I practice protective documentation; focusing strictly on clinical interventions and necessary compliance rather than recording raw personal disclosures, private stories, or unnecessary details that could be weaponized against you.
While formal diagnostic codes are required for administrative compliance or institutional advocacy, I do not use labels to define your humanity. Diagnoses and charted life stressors are chosen strategically to serve your direct interests; whether securing accommodations, navigating legal systems, or protecting your access to care. Stressors are only named when doing so serves a legitimate purpose. There is more information about this topic explicityly in Accommodations & Inaccessible Systems. It makes more sense to talk about charting in this lens there because documentation is fundamentally a form of advocating in an innaccessible and pathologizing system that often jumps straight to compliance and medication.
Intake forms and biopsychosocial assessments are placed directly in your hands. You choose what information to fill in. This fulfills regulatory compliance while respecting your personal boundaries. Our work is built on mutual trust rather than investigative scrutiny. I rely on what you choose to share, and if incongruence arises in your story, I am confortable discussing that directly. It is not awkward, it is normal. My charting is your friend.
This commitment to privacy connects directly to our overall practice information, including my financial model and direct contact channels.
